Subtotal parathyroidectomy for the treatment of persistent hyperparathyroidism after kidney transplantation
Abstract
This project aims to improve the treatment of disturbed mineral metabolism in kidney transplant recipients. Despite improvement in kidney function following a kidney transplantation, a substantial number of patients suffer ongoing disturbances of mineral metabolism, with persistent hyperparathyroidism in 70% of patients 1 year after kidney transplantation. These disturbances may convey negative effects on long-term health, however, guidelines on treatment options are lacking, leaving many patients untreated.
This project will investigate the safety and efficacy of subtotal parathyroidectomy for treatment of persistent hyperparathyroidism after kidney transplantation. First, we will utilize the comprehensive Danish health-registries to investigate long-term outcomes of a parathyroidectomy in a cohort of kidney transplant recipients. Second, we will investigate safety and efficacy of a parathyroidectomy in a prospective cohort study. Finally, we will conduct an intervention trial with the intent to compare the effect of subtotal parathyroidectomy to usual care in kidney transplant recipients with persistent hyperparathyroidism. The combined knowledge from this project has the potential to change the treatment standard of persistent hyperparathyroidism after kidney transplantation and ultimately aims to improve musculoskeletal health for kidney transplant recipients.
